Trans Abstract

Wide-awake surgery is a surgical procedure which is not used a tourniquet and any sedation by using the function of lidocaine and epinephrine through the tumescent injection technique. The biggest advantages of wide-awake surgery are the prevention of the systemic risk of general anesthesia and the surgeon can confirm the result of surgery by moving the patient’s joint and tendon during the operation. But some modification of wide-awake surgery may be beneficial for the surgeon and patient. For decreasing the patient anxiety, low-grade sedative can be used and short-time use of tourniquet during initial dissection of the operating site can prevent the unexpected complication.

서론

각성 수술(wide-awake surgery)은 진정제 사용 없이 정상적인 의식을 가진 환자에게 리도카인(lidocaine)과 에피네프린(epinephrine)을 섞어 필요한 절개 부위에 주사하는 국소 마취만으로 수술 통증은 막고, 지혈대 사용 없이 수술 시야에 출혈이 없는 상태를 유지하는 수술 방법이다[1,2]. 각성 국소 마취 및 무지혈대(wide-awake local anesthesia with no tourniquet, WALANT) 수술로도 알려져 있다.

기존의 전신 마취나 상완신경총 차단 마취는 수술 중 환자의 통증을 없애는 것은 가능하나 운동 신경까지 같이 마비되면서 수술 부위의 능동운동을 확인할 수 없는 단점이 있다. 수술 중에도 운동 기능을 유지할 수 있는 정맥 마취(Bier anesthesia)는 지혈대 통증으로 인한 마취 시간의 제한 및 리도카인의 전신 유입 등의 문제로 그 사용에 제한이 있다. 이러한 마취들은 수술 전 환자의 전신 건강 상태에 대한 검사와 수술 후 회복을 위한 안정과 관리를 위하여 입원이 필요하며, 더 많은 의료용 재료가 필요한 단점들이 있다.

수술 후의 운동 회복이 특히 중요한 상지, 특히 수부 수술에 각성 수술을 적용하면, 수술 중 건의 움직임과 관절의 능동적인 움직임을 확인하고 필요에 따라 술 중 바로 교정할 수 있다. 그리고 수술된 조직의 장력과 강도가 적절한지 실시간으로 확인할 수 있으므로 수술 중에 수술 결과를 예측할 수 있으며, 보다 빠르고 적절한 물리치료가 가능하다[3,4]. 또한 각성 상태의 환자는 수술 중 의료진과 원활한 의사 소통을 통해 수술 부위 착각의 여지를 근본적으로 예방할 수 있고, 수술 결과를 환자가 수술 중 확인하면서 치료자와 신뢰 구축에 긍정적인 효과까지 있다. 이러한 장점들로 인해 더 우수한 수술 후 결과가 보고되고 있는 매우 유용한 수술 방법이다[5].

기존에 발표된 각성 수술은 (1) 환자는 수술 전 과정에서 각성 상태를 유지하고, (2) 지혈대를 사용하지 않고, (3) 마취과 의사가 마취에 관여하지 않고 외과의(surgeon)가 직접 마취를 하며, (4) 환자는 수술 중에도 의사의 지시에 따라 손가락과 손, 손목까지 자유롭게 사용할 수 있어야 한다는 것을 조건으로 하고 있다[6].

처음에는 수술 중 능동운동 확인이 중요한 수부내 건에 대한 수술에 주로 사용되어 왔으나, 점차 골절, 신경, 관절 수술이나 더 근위부의 상지 수술로 그 적용 범위가 확대되고 있다. 최근 수부 각성 수술 적용 부위는 일차 또는 이차 건 재건술, 수근부 내시경[7], 압박성 말초신경 신경병증 유리술, 중수골, 수근골 및 원위 요골 및 척골 골절의 고정, 대다각골 절제술, 듀피트렌 구축(Dupuytren contracture)의 수술에도 사용되고 있다[6,8]. 에피네프린의 사용으로 상대적 금기증으로 생각되던 피판술이나 미세 접합술[9]에서도 성공적인 사용이 보고되고 있으며, 전족부와 발목[10]에서도 그 사용이 보고되고 있다.

이 글에서는 기존의 각성 수술 방법을 고찰하고, 적용되는 환자의 특성과 국내 의료환경에 적절한 각성 수술의 변형에 대하여 이야기해 보고자 한다.

수부 각성 수술

1. 적응증

수부 수술이 필요한 환자 대부분(95%)에 적용 가능하나 어린 소아, 복합 부위 외상(major trauma) 환자, 의식의 변화(mental change)가 있거나 정신적인 문제로 협조가 불가능한 환자에게는 적용할 수 없다. 특히 주사 바늘에 대한 극단적인 공포심(phobia)이 있거나, 수술 시 극단적인 불안증을 느끼는 환자 또한 상대적인 금기증에 포함해야 한다. 국소 마취제에 대한 과민반응 또한 금기증에 포함되는데, 이는 두 가지 형태로 분류된다. 즉, 접촉 피부염(contact dermatitis) 증상, 또는 부종의 소견으로 나타나는 지연성 4형 과민반응과, 1형의 아나필락시스(anaphylaxis) 과민 반응이 발생할 수 있다[11]. 4형 반응 발생은 0.7%에서 2.4%로 보고되며[12], 1형은 매우 드물게 발생된다[13].

2. 마취

리도카인은 부피바카인(bupivacaine)의 심장 독성에 비하여 비교적 안전한 마취제로, 안정 용량은 7 mg/kg로 알려져 있다. 체중 70kg의 성인에서 안전 용량은 1% 리도카인 50 mL 정도이다[14]. 대부분의 2, 3개 수지의 수술은 보통 50 mL보다 작은 용량으로 마취가 가능하다. 이러한 경우는 1% 리도카인 용액 20 mL에 1:100.000 에피네프린 0.2 mL를 섞어 20 mL 주사기에 준비한다(Fig. 1).

리도카인이 산성을 가지고 있으므로 중화를 위하여 8.4% 탄산수소 나트륨(sodium bicarbonate) 2 mL (10:1 비율)을 섞어주면 주사 시 발생하는 통증의 조절에 도움이 된다[15]. 전체 리도카인 주사량이 7 mg/kg를 넘게 되는 3개 이상의 수지 굴곡건 수술이나 손목 및 전완부 수술과 같이 넓은 수술 부위에 50 mL 이상의 국소 마취제를 사용할 경우에는 생리식염수로 희석해서 0.5% 리도카인, 1:200.000 비율의 에피네프린 사용이 추천된다. 만일 수근부보다 근위부의 넓은 부위를 마취한다면 100–200 mL의 대량의 주사가 필요할 수 있다 이러한 경우에는 0.25% 리도카인과 1:400,000비율의 에피네프린 사용도 가능하다(Table 1) [16].

마취제는 가능한 신체온도에 맞추어서 준비하면 좋다. 리도카인과 에피네프린의 최대 효과가 발현되는 데에는 최소 26분 이상이 필요하므로[15], 피부 절개 최소 26분 이전에 주사해야 한다. 국소 주사 시 통증을 줄이기 위해 권장되는 방법은 다음과 같다.

  • (1) 가능하면 가는 주사 바늘을 사용한다(27 gauge, 0.41 mm 이하) [17] (Fig. 1).

  • (2) 주사기를 잡을 때 양손을 모두 사용하여 바늘이 흔들리지 않도록 한다.

  • (3) 바늘이 피부를 관통할 때 피부에 수직으로 삽입하여 접촉되는 피부 신경을 최소화한다[18].

  • (4) 바늘로 피부를 찌르지 말고, 피부를 밀어 올려 피부가 바늘로 들어가게 하면 통증을 줄일 수 있다. 주변 피부의 작은 진동이나 압력 등은 바늘이 피부를 관통할 때 초기 통증 감소에 도움이 된다.

  • (5) 2–10 mL의 마취액을 피하에 주사할 때 바늘이 움직이지 않도록 주의해야 한다.

  • (6) 넓은 범위를 마취할 때는 이전 주사로 피부 색이 하얗게 변한, 감각이 둔해진 부위 1 cm 이내에 다음 바늘을 삽입하여 통증을 줄여야 한다[19].

  • (7) 마취액은 충분한 양을 주사해야 한다. 단 손가락 수장부와 수배부는 각 수지(phalanx)에 2 mL면 충분하다(Fig. 2).

  • (8) 마취 범위는 절개 부위나 골절 부위에서 최소 2 cm 이상의 폭으로 충분히 마취액이 들어가야 한다.

  • (9) 마취 중 환자와 원활한 의사소통으로 통증 여부를 확인하여, 최소한의 통증으로 마취가 되도록 꾸준히 연습(training)하여야 한다.

  • (10) 전완부 건 이전술(tendon transfer) 등 광범위한 마취가 필요한 경우는 Blunt tipped filler cannula를 사용하여 주사하는 것이 도움이 된다[8].

과거 괴사에 대한 우려로 사용이 금기되어왔던 수지 내 에피네프린 사용은 그동안 여러 연구에서 그 안정성이 증명되었다[20,21]. 하지만 최근에도 에프네프린 사용 후 수지 괴사 발생이 보고된 바가 있어[22], 에피네프린 주사 후 수지 허혈 상태가 지속될 경우 상황에 따라 알파 차단제인 펜톨아민(alpha blocker phentolamine) 1 mg을 1–10 mL의 생리식염수에 섞어 허혈 부위에 주사하면 혈관 수축을 풀어줄 수 있다(Fig. 3). 이때 펜톨아민 주사 위치는 기존 마취제 주사와 동일한 위치에 주사하면 효과적이다[23].

3. 수술

첫 마취 용액 주입을 시작하고 30분 이후, 또는 모든 마취를 종료한 시점으로부터 15분 이후에 수술을 시작한다. 에피네프린은 최소 15분의 시간이 지난 이후에 정상적인 지혈 작용을 할 수 있기 때문이다[24].

지혈대를 이용하는 다른 마취 방법과 달리 작은 수술방이나 외래 수술실에서도 수술 준비가 가능하며, 수술 부위 소독 범위 또한 더 작게 준비할 수 있다. 수술 절개 중 약간의 출혈이 있더라도 전기소작을 시행하지 않고 거즈로 누르거나 가볍게 닦아내면 대부분의 출혈은 멈추게 된다.

수부 각성 수술의 적응증

1. 굴곡건의 치료

수부 각성 마취가 가장 광범위하게 적용되고 있는 영역으로, 굴곡건 파열의 봉합, 유착의 박리, 진구성 손상 시 건 이식이나 건 전이술 등의 굴곡건 수술에서 각성 수술을 이용하면서 재파열률의 감소와 유착으로 인한 재수술의 감소가 보고되고 있다[25]. 저자의 경우 굴곡건의 수술, 특히 이식이나 전이술에서 수부 각성 수술 방식 이외의 전신 마취나 상완신경총 차단 마취는 더 이상 사용하지 않고 있다.

굴곡건 봉합 수술 중 시행하는 능동적인 ‘수지 굴곡 신전 검사(digital extension-flexion test)’는 봉합 후 힘줄 활주에 걸림을 파악할 수 있을 뿐만 아니라, 능동적인 운동을 통하여 봉합 부위에 결손(gap formation)이 발생하는지 확인이 가능하다. 이 검사를 통하여 적절한 강도의 굴곡건 봉합 여부를 바로 판단할 수 있고, 수지를 완전히 굴곡·신전하게 함으로써 굴곡건 활차가 봉합되어 굵어진 굴곡건의 활주에 방해가 되는지 확인할 수 있다. 만일 운동 시 걸리는 현상이 있거나 운동이 완전히 되지 않으면 활차를 추가적으로 열어줄(venting) 수 있다(Fig. 4) [25].

천수지 굴곡건의 봉합 여부 또한 아직까지 확립된 것은 없으나, Tang 등[25]은 (1) 손상 부위가 깨끗한 열상일 때, (2) 봉합이 용이할 때, (3) 파열 부위가 A2 활차에 위치하지 않을 때 등에 천수지 굴곡건 봉합을 추천하였다. 그러나 (1) 지연된 일차 봉합 시(delayed primary repair), (2) 파열 부위가 A2 활차에 위치할 때, (3) 천수지 굴곡건이 근위부로 전위가 심할 때는 천수지 굴곡건을 봉합을 추천하지 않는다고 하였다(Fig. 5).

굴곡건 재건을 위하여 각성 수술을 이용한 건 이전술을 시행할 때는 수장부의 해부학적 특수성과 유착으로 인한 합병증 발생에 주의해야 한다. 이전된 건의 긴장도(tension)와 봉합 강도를 수술 중 확인할 필요가 있을 때까지 지혈대의 적절한 사용과 얕은 진정(sedation)을 시행하는 것도 추천된다(Fig. 6) [26].

1구획이나 3구획 근위부 손상에서의 굴곡건 봉합은 2구획에서의 원칙과 동일하며, 특히 1구획에서는 봉합 부위와 매우 가까운 위치에 A4 활차가 존재하므로 대부분의 경우 A4 활차의 완전한 절개(vent)가 필요하다. 또한 국소 팽창 주사(tumescent technique)의 경우 수술 부위 부종으로 수술 후 봉합부의 감염이나 벌어짐이 있을 수 있어, 통상적으로 3일간 안정 후 운동 및 물리치료를 시작한다

2. 신전건의 치료

수지 내에서 신전건의 봉합이나 재건술 시 신전건 자체가 얇고 약하여, 수술 중 수지의 무리한 굴곡을 시행하면 봉합 부위의 파열을 초래하는 경우가 많으므로 주의를 요한다. 하지만 단추 구멍의 변형이 발생된 3구획의 봉합이나 건 재건술 시에는 적절한 강도의 봉합을 얻었는지 또는 관절의 완전한 운동이 가능한지 확인이 필수적이며, 건의 적절한 긴장도를 맞추기 위해서는 수술 중 환자의 능동적 운동을 확인하는 것이 가장 정확한 방법이어서 각성 수술을 필수적으로 사용해야 한다. 장무지 신전건 지연 파열 시의 재건술이나 중수지관절 시상대(sagittal band) 파열의 치료에도 각성 수술을 사용하면 효과적이다(Fig. 7). 건 이전술로 신전건을 재건할 경우, 건의 긴장도가 부족하거나 과도하면 적절한 수지 운동의 회복이 불가능하다[27]. 따라서 수술 중 긴장도가 적절한지 확인 가능한 각성 수술의 사용은 무엇보다 중요하다. 요골신경의 마비로 발생한 모든 수지신전 마비의 경우에도 전완부 각성 수술로 성공적으로 신전건을 재건할 수 있다(Fig. 8) [28].

3. 건 박리

건 유착은 외상이나 수술로 건과 주변 조직 사이의 흉터 조직이 형성되면서 발생한다. 이러한 유착은 주변 관절의 운동에 영향을 미치게 된다. 건 박리술만으로 수지운동 범위의 상당한 회복을 얻을 수 있으나, 완전한 운동을 위해서는 관절낭 유리술이나 절제, 측부 인대의 절제 또는 수장판의 유리술이 필요한 경우가 많다. 수술 중 환자의 능동운동을 확인함으로써 수술의 적정성 여부를 최종 평가하고 예후를 예측할 수 있다(Figs. 9, 10) [29].

4. 골에 대한 치료

모든 수지에 국한된 골절은 각성 수술의 적응증이 될 수 있지만 동반 손상으로 동맥 손상이 있거나 정맥 울혈이 의심되는 경우, 그리고 아주 불안전한 골절이나 관절의 운동을 막는 고정 방법을 사용해야만 하는 골절에는 굳이 각성 수술 방법을 고집할 필요는 없다.

정복 후 비교적 안정적인 간부나 골간단부 골절의 경우, 근위지 관절의 중위지골 기저부 골절 중 역동적 외고정으로 치료를 결정한 경우, 그리고 근위지골의 전방각 형성 간부 골절 중 굴곡건이 골절 끝에 걸리면서 건 포착이 예상되는 경우에는 각성 수술이 적극적으로 필요하다(Fig. 11).

각성 수술 하에 고정 후 운동을 하여도 전위가 없어서 빠른 재활운동이 가능한 경우는 술 후 3–5일 간 immobilization을 시행한 뒤 30도 정도의 관절운동을 수동적, 능동적으로 시행한다. 최소한 5 mm의 심수지 굴곡건 활주(gliding) 운동을 할 수 있으면 건 유착의 방지에 효과적이다. 이때 골절 정복의 소실 여부와 강선 고정 부위의 감염에 대해 항상 주의하고 교육하며 운동을 진행하여야 한다[8].

수근부와 전완부의 골절에서도 수근부로 가는 3개의 주요 신경 부위를 국소 마취함으로써 연부 조직뿐만 아니라 골에 통증을 지배하는 신경의 마취가 가능하여 각성 수술이 적용되고 있으며, 현재는 쇄골의 골절 내고정에도 각성 수술 사용이 보고되고 있다.

각성 수술의 장점

수술 중 손가락이나 상지관절의 운동을 의사와 환자 자신이 바로 확인하고, 적절한 능동운동이 되지 않을 때 원인을 바로 확인하여 해결 가능하다는 점이 가장 큰 장점이다. 수술의 결과에 대하여 환자와 의사가 명확한 의식 하에 운동을 확인하여 향후 재활 치료에 대한 교육을 할 수 있고, 운동 시 수술 부위 재파열의 두려움을 감소시킬 수 있으며[30], 상호간의 신뢰 구축에 큰 도움이 된다는 것 또한 장점이 된다.

또한 비교적 경제적이며, 안전하다. 특히 서구 여러 나라에서 시행되는 포괄수가제 정책 하의 의료 환경에서나, 적절한 의료장비가 부족한 개발도상국가에서 전문적인 마취과의 도움이나 장비가 없는 경우에도, 비교적 장시간의 수술을 간단한 국소 주사만으로 시행할 수 있어 효과적이다. 사전 입원이나 수술 후 회복을 위한 입원 치료가 필요 없어 의료 비용이 절감되며, 다른 마취 방법에 비하여 마취 작용 시간이 길어 술 후 진통제 사용량도 줄일 수 있다. 수술로 발생되는 의료 폐기물의 발생을 줄여 환경 보호에도 긍정적인 효과가 있다[31].

Wide-Awake Local Anesthesia No Tourniquet의 변형

1. 수술 중 각성

각성 수술은 내과적인 문제가 있는 환자에서 “The safest sedation is no sedation”라는 문구와 같이 안정성 면에서 장점을 가지고 있지만, 환자의 성향에 따라 수술실 내로의 진입과 수술대에 누워있는 상황에 대한 과도한 불안감을 호소하는 등 장시간의 수술을 진행하기 어려운 경우가 있다. Teo 등[32]의 연구에서도 수부 각성 수술 중 15%의 환자에서 진정이나 완전한 수면이 필요했다고 보고하였다[32]. 수술을 시행하는 의료진 또한 감정의 동화가 발생할 수 있으며, 건 전이술이나 건 이식 수술 중 적절한 건의 긴장도를 얻기 위하여 건을 당기면, 마취된 절개 부위보다 근위부의 근육에 과도한 통증을 호소하거나 근육이 긴장되어 오히려 적절한 긴장도를 얻거나 평가하기 어려운 경우가 있다. 이러한 경우 상황에 맞게 마취과 의사의 협조 아래 시행한 적절한 얕은 진정(weak sedation)은 환자의 정신적인 고통을 줄여줄 수 있으며, 마취제의 불필요한 추가 사용을 줄일 수 있어 무조건 “no sedation”을 고집할 필요는 없다. 저자의 경우 상완신경총 차단 마취를 통하여 수술을 하는 경우에도 진정을 원하는 환자의 빈도는 30%가 넘었다. 수술 과정 중 각성 수술의 주요한 이유가 되는 환자 운동성의 확인이 이루어진 이후, 특히 여성의 경우 환자가 원한다면 불안을 없애고 진정 효과를 얻기 위해 benzodiazepine 같은 약한 진정제(sedative)를 투여하여 환자의 수면을 유도한다. 만일 환자의 협조나 능동운동이 필요할 때나 의사의 설명을 들어야 할 때는 언제라도 환자를 다시 깨우면 더 나은 환자 만족도를 얻을 수 있었다[28].

2. 지혈대의 사용

각성 수술 시 주사 통증을 줄이고 출혈을 막기 위해 주변에 충분히 팽창(tumescent) 방법으로 주사를 하게 된다. 마취액으로 부어있는 연부 조직을 절개하면 마취액이 누출되고, 여기에 불가피한 소량의 출혈이 발생되면 수술 부위의 해부학적인 경계를 명확하게 파악하기 어렵다. 수부, 특히 수장부 수술의 경우 피부가 두껍고 신경이나 혈관의 해부학적 구조가 복잡하며, 재수술의 경우 심한 유착이 발생되어 있는 경우가 많아, 수술 초반 절개 시 약간의 출혈이나 조직의 부종은 중요 구조물의 손상을 일으킬 위험성이 높다[26]. 개인에 따라 다르겠지만 일반적으로 20분의 상박부 지혈대 적용(본원의 경우 상지의 경우 200 mmHg 적용)은 견딜 수 있다[33]. 피부 절개 직전에 지혈대를 적용하고 무혈 상태로 초기 박리를 진행하여 적절한 수술 대상을 확인 및 분리한 후 지혈대를 풀고 불필요한 출혈을 지혈한 후 건 봉합이나 골 고정 등 목표한 수술을 마무리하는 것이 좀더 안전한 방법이라 생각된다. 이러한 변형은 “wide-awake local anesthesia with a brief tourniquet period”라고 명명할 수 있다. 각성 수술 시 수술 중 언제라도 출혈로 인해 수술 시야가 제한되거나, 이로 인한 주요 구조물 손상이 걱정되는 경우, 또는 예상치 못했던 대량 출혈이 발생되는 경우에 지혈대를 다시 사용하여 짧은 시간에 출혈을 막는 것은 수술 과정의 효율성 면이나 환자에게 불필요한 해를 주지 않는 점에서 장점이 있어, 굳이 무지혈대 방식(no tourniquet)을 고집할 필요는 없다. 저자는 모든 각성 수술 시 공기 지혈대를 상박에 감고 수술 준비를 한다[28].

3. 마취자

각성 수술 마취는 모든 의사가 간단한 교육을 통하여 쉽게 시술 방법을 습득할 수 있다. 복잡한 해부학 지식이나 마취과 지식을 갖추지 않더라도, 필요한 수술 부위에 정확한 주사 시술 원칙을 지킨다면 성공적인 마취 결과를 얻을 수 있다. 특히 수술을 계획하고 전담할 주치의가 직접 주사를 하게 되면 더 정확한 마취 결과를 얻을 수 있을 것이다. 하지만 저자의 병원에서는 마취과 의사가 각성 수술 마취를 전담하고 있다. 물론 마취의는 주사 시행 전에 주치의와 면밀한 협의를 통해 정확한 마취 범위를 확인한다. 마취과 의사가 각성 수술 마취를 전담하는 이유는 각성 수술 마취의 또 하나의 장점인 수술실 사용의 효율성과 사용률(utility rate)을 높이기 위해서이고, 수술을 전담하는 의사는 수술에 전념할 수 있어 수술의 집중도 향상에도 도움이 된다. Lied 등[31]의 연구에 따르면 각성 수술 시 환자의 수술실 체류 시간이 다른 전신 마취나 액와 마취보다 더 길며, 수술자가 직접 마취하면 수술실 회전율은 오히려 떨어진다고 보고하였다.

넓은 부위에 마취를 시행할 때 초음파를 이용하여 감각신경만 선택적으로 마취하면, 마취제 사용도 줄이고 보다 안전한 마취를 할 수 있다[34].

결론

각성 수술은 수술 과정과 결과를 수술 중 평가할 수 있고, 발생하기 쉬운 합병증을 예방하고 의사와 환자 간의 신뢰를 형성할 수 있는 방법이다. 수술 초기의 낮은 단계의 진정제 사용과 짧은 시간의 지혈대 사용 등 각 수술실 환경과 환자의 특성을 고려한 변형된 방법의 각성 수술 적용을 통하여 보다 성공적인 수술 결과를 얻도록 노력하여야 할 것이다.
