Abstract
Purpose
The purpose of this study is to review the clinical results of open debridement arthroplasty using the modified Outerbridge-Kashiwagi procedure through posterior approach on the osteoarthritis of elbow. Materials and Methods: The patients were chosen from who had open debridement arthroplasty using the modified Outerbridge-Kashiwagi procedure through posterior approach after being diagnosed of primary or secondary osteoarthritis of the elbow from July 1995 to July There were 23 men and 6 women. Mean age was 42.3 (27~63) years. Eight cases had trauma history. Elbow joint was opened through medial and lateral side of triceps tendon. Ulnar nerve was transposed to anterior side of elbow joint. At first, the loose bodies and osteophytes of posterior aspect of elbow joint were removed and then 1.5 cm hole diameter, penetrating from olecranon fossa to coronoid fossa, was made. Through the hole, the loose bodies of coronoid fossa and osteophytes of coronoid process were removed and the anterior capsule was released. Average period of follow up was 4 years and 8 months (3 months~10 years and 3 months). Statistical analysis was performed on pain, range of motion, returning to work, subjective satisfaction with work performance and improvement of ulnar nerve symptom before the operation and at the last follow up. Pain was assessed by using visual analogue scale, Result: We found out that patients’ condition was improved. Pain was improved from 7.1 points to 3.7 points (p<0.001). ROM was improved from 80±27°to 98±18°(p<0.001). Most patients returned to their work, and the subjective evaluation on work performance was improved from 30.5 (22~36) points to 67.3(57~74) points (p=0.001). The ulnar nerve symptom was disappeared after operation in all 7 cases. Conclusion: Open debridement arthroplasty using modified Outerbridge-Kashiwagi procedures through posterior approach reduces pain and improves range of motion and is recommendable treatment option in primary and secondary elbow osteoarthritis.
