The Effects of Proximal Nerve Compression on Neural Regeneration Following Distal Nerve Repair in the Rats

Abstract
Purpose
To evaluate the effects of proximal nerve compression on neural regeneration following the distal nerve repair on a sciatic nerves of rats. Materials and Methods: A 5 mm tendinous band made of the patellar tendon harvested in each rat was placed around the sciatic nerve to make constriction band with internal diameter 0.2 mm (group I) or 0.6 mm (group II) greater than diameter of the sciatic nerve. At 3 months after nerve cuffing, we assessed the degree of nerve compression and performed distal transection and repair, dividing all rats into six subgroups according to cuffing and repair; A, control; B1, from group I; B2, from group II; C, transection and repair; D1, transection and repair in group I; D2, transection and repair in group II. At 6 months after nerve repair, electrophysiologic study, histologic study and morphometric nerve fiber analysis were performed . Results: At 3 months after nerve cuffing, mild nerve compression was noted in group I while findings were similar to normal control in group II. At 6 months after nerve repair, axon and nerve diameters were significantly decreased and nerve fiber density was significantly increased in C, D1 and D2 groups. The nerve fiber density was similar between both C and D2 groups. However, the nerve fiber density was significantly decreased in D1 group in comparing with C and D2 groups. In electrophysiologic study, there was no statistical difference among C, D2 and A groups. But statistical difference was found in D1 group comparing with A group. Conclusion: This study showed that proximal nerve compression has the negative effects on neural regeneration following nerve repair in the distal sciatic nerve of rats.
