After follow up of 8-12 months, the range of motion of the finger was measured and compared with opposite normal finger. We used the Criteria of Crawford as an parameter of the results. Result: The union of the fractures were completed in 4.7 weeks in average. The excellent was 16 cases, good was 5 cases and fair was 2 cases. There was 1 case of fracture displacement and 2 cases of pin site infection. Conclusion: We performed direct percutaneous reduction, compression and K-wire pinning of the mallet fracture. If we carefully reduce and insert the trans-fracture K-wire, we can get the good results without injury of soft tissue and extensor mechanism
