Bone-Extensor Carpi Radialis Longus Tendon Ligamentoplasty for Chronic Radial Instability of the Thumb Metacarpophalangeal Joint: A Case Report

Abstract
One case of chronic instability of the radial collateral ligament of the thumb metacarpophalangeal joint is presented. This patient was treated using the bone-extensor carpi radialis longus tendon ligamentoplasty. The bone block was impacted and secured at the point of origin of the radial collateral ligament with a 1.5 mm cortical screw. The principal tendon portion was inserted into the base of the phalanx with Mitek 2.0 mm Tacit threaded anchor and 3-0 Ethibond suture under moderate tension. A stable thumb metacarpophalangeal joint was achieved in the case with no recurrent instability or pain found within 6 months of follow-up. Grip and pinch strength and range of motion were nearly the same as in the uninjured hand. The importance of correct anatomical placement of the anchor was stressed.
