Prognosis of Carpal Tunnel Release in Patients with Extreme Thenar Atrophy

Abstract
Purpose
: We evaluated the clinical and functional recovery of thenar muscle function in patients with extreme carpal tunnel syndrome after surgical release. Materials and Methods : 30 hands in 26 patients who had severe thenar atrophy and were unresponsive to the motor nerve conduction study were studied. Clinical grade of thenar atrophy, patient’s satisfaction, time to recovery and the recovery of the motor nerve conduction were investigated. Results : Twenty-nine of thirty hands had full recovery or mild persistence of thenar atrophy. Twenty-nine hands were satisfied with their results. The average recovery period was 6.9 months, ranging from 3 months to 14 months. The follow-up motor nerve conduction study showed persistent no motor evoked potential in 2 hands. Remaining 28 hands showed responses with the average latency period being 4.4±1.254 m/sec and the average amplitude being 9.8±11.398 mV. There were no significant relationships comparing between the patient’s age and duration of symptom with degree of recovery. Conclusion : In patients with extreme carpal tunnel syndrome accompanied with thenar atrophy, high rate of recovery of motor function is expected. Opponensplasty during surgical release of extreme carpal tunnel syndrome is rarely indicated.
