Temporary Intramedullary K-wire Fixation for Metacarpal Neck and Phalangeal Fractures

Abstract
The conservative management of metacarpal and phalangeal fractures has been associated with various complications such as joint stiffness, angular and rotational deformity. Authors analyzed a retrospective review of 50 digits in 38 patients with distal metacarpal and phalangeal fractures treated by temporary intramedullary pinning and cross or oblique K-wire fixation for prevention these complications. The results were evaluated on the basis of categories of clinical outcome assessment proposed by Belsky. Forty-five cases showed excellent results, four cases had limitation of motion and one case was reoperated due to nonunion. We recognized that temporary intramedullary pinning with cross or oblique fixation was one of simple and useful methods for treating distal metacarpal and phalangeal fractures, and good results could be obtained in the majority of appropriately selected patients.
