Proximal Row Carpectomy for Management of The Post-Traumatic Arthritis of The Carpus

Abstract
Purpose
: This study reports the outcome of proximal row carpectomy for management of the moderately posttraumatic arthritis of carpus and evaluates surgical technique and pitfall, limitation with use. Material and Methods : Six wrists treated for posttraumatic arthritis of the carpus between 1996 and 2001 년 with proximal row carpectomy were retrospectively reviewed at an average of 2.6 years postoperatively. Etiology included: scaphoid nonunion with arthritis in 2 cases, perilunar dislocation with arthritis in 1 case, was indeterminate with arthritis or severe injured in 3 cases. Evaluation of the results was based on X-ray film, Preoperative and postoperative pain, activity level, the range of motion(ROM) and grip strength. Results : Review of preoperative x-ray films revealed that 2 cases had SNAC(scaphoid nonunion advanced collapse), 3 cases had stage II SLAC(scapholunate advanced collapse) and 1 case had stage III SLAC. The subjective results were excellent in 2 cases, good in 3 cases, fair in 1 case. The total arc was improved from average 5 degree to 67 degree. The grip strength using dynamometer was increased from average 6.5 lb to average 23.6 lb. 3 cases(50%) returned to previous occu-pation and none have had to restrict their daily activity in 4 cases. Conclusion : Proximal row carpectomy is a good reconstructive treatment option for management of the post-traumatic arthritis of carpus because the procedure also appears to diminish pain, maintains a functional arc of motion, and improves grip strength.
