Improvement of Function in Traumatic Stiff Proximal Interphalangeal Joint with Hinged Dynamic External Device

Abstract
Stiffness of the joint is a well known complication of proximal interphalangeal joint of the finger after hand trauma, especially in fracture and fracture dislocation. Not only current bone and joint injuries but also soft tissue trauma and persistent edema can produce stiff joints. Unfortunately, most post-traumatic stiff proximal interphalangeal joint management outcomes were not so satisfactory, in spite of many kinds of treatment options. Author experienced 6 cases of long standing post-traumatic stiff proximal interphalangeal joints management with recently introduced hinged external fixation device with aggressive physical therapy. All of the cases were chronic stiff joints in adults, average follow-up was 12months. During follow-up period we achieved average 60 degree joint motion compared with before management. There was no significant complication. We observed the tendency of the gradual decreasing joint motion during follow-up after external hinged device. We can conclude this method is safe and a minimally invasive technique with good results if the indication is proper and patients are highly motivated. We propose this method in the patient with proximal interphalangeal joint stiffness
