The nail of the thumb is one of the frequent sites in the occurrence of the malignant melanoma and requires early diagnosis and excision at 1- 3 cm from the lesion in recent trends. We excised the nail of the thumb and soft tissues of the distal and proximal phalanx in a radical dissection fashion t o the metacarpophalangeal j oint level leaving distal and proximal phalangeal bone, flexor and extensor tendons and intrinsic ligament s int act and wrap- around free flap was chosen for the reconstruction of the thumb. We report a case of thumb reconstruction in malignant melanoma with wrap- around flap from the big toe and followed up 5 year s and 5 months.
