Abstract |
From February 1991 t o August 1997, author s had performed hand bone lengthenings in the 16 patient s, 19 cases based on callot asis using ext ernal fixat or. Average age at the time of surgery was 23 years old(from 4 t o 56 year s old). Diagnoses included traumatic finger amputations in 11 cases, congenit al hypoplastic digit s in 8 cases. Sit es of lengthening were metacarpal bone in 12 cases, proximal phalangeal bone in 7 cases. 19 cases of all except two cases obt ained complete consolidation at the lengthening sit e. In one case secondary bone graft was required due t o bone defect , in another one case absorbable biofixat or insert ed additionally aft er removal of ext ernal fixat or due to incomplete bone union. Average gained length was 16.6mm(from 6 t o 30mm) and average percent of gained digit al bone length was 54.8%. Average consolidation period was 15 weeks(from 10 t o 29 weeks) and average healing index was 52.7 days/ cm. There were minimal complications including transient numbness, marginal necrosis of skin, bone tip exposure and flexion contracture were responded t o conservative manage, but stump revision was performed in one case of bone tip exposure. In all cases, there was improvement of function of hand with key pinch and cosmetic aspect . In conclusion, the callot asis lengthening using external fixat or is one of the useful method of short hand bone lengthening to improve function and cosmetic demand. |
