The long term follow-up result of revascularization in treatment for Kienbck's disease

Abstract
Revascularization, composed of direct implantation of vascular loop, pronator quadratus muscle pedicled bone graft, etc., is the procedure which restores the blood supply in an avascular necrotic lunate. We performed two operative procedures, microvascular loop graft and pronator quadratus muscle pedicled bone graft, in six avascular necrotic lunates of stage II or III-A. The long term follow-up result of these procedures was not reported yet. So we purposed the establishment of the usefulness of these procedures in reestablishment of blood supply in necrotic lunates. We have three microvascular loop graft under microscope and three pronator quadratus muscle pedicle bone graft. We adapted Mayo wrist score to assess the clinical results. The result was three excellent, two good and one fair. The patients were free of pain and regained nearly full range of motion in wrist joint. The radiographic finding was not deteriorated with elapse of time. We conclude that the revascularization(microvascular loop graft and pronator quadratus muscle pedicle bone graft) is the promising procedure of the restoration of blood supply in stage II or III-A of Kienb ck's disease.
